Bemidji State Beavers Athletics

Below is all the information needed for new/incoming student-athletes. This includes freshman and transfers.
 
Physical Examination Requirements: All athletes must have pre-participation physicals complete in order to compete in collegiate athletics here at BSU. All newcomers must have their physical completed within six (6) months of the BSU’s academic start date. Returning athletes must update their physicals yearly for them to remain valid. Please use the forms provided when visiting your healthcare provider for your physical examination. This needs to be uploaded to ATS, you can do this or give it to your athletic trainer. Sickle Cell Trait Test Mandate: The NCAA mandates all student-athletes participating in an on campus tryout have knowledge of their sickle cell trait status. Athletes have the following options: 1) show proof of sickle cell testing done at birth 2) have a physician conduct a new blood test to check for the sickle cell trait and provide results. BSU student athletes will provide this information on their physical form.

Attention Deficit Hyperactivity Disorder: If you have been diagnosed with ADHD and are currently taking medication the NCAA requires you to have your attending physician fill out the following document and provide the required documentation. The document is needed in case you are selected for random drug testing while participating in intercollegiate sports. This needs to be uploaded to ATS, you can do this or give it to your athletic trainer. Athletic Trainer Systems (ATS): ATS is used by the Athletic Training Staff to document injuries, illness, rehabilitation, insurance and contact information. This is also where our yearly forms are located. Below you will see the link to ATS, what needs to be completed and a How to PDF and video to help you. Concussion Testing: SWAY is completed using an application on your phone. Please have it downloaded before starting the test. Below is the link for the application.
